In addition to the online application, the applicant must do the following:
Print and sign the Signature Page which comes up after the application has been submitted.
Compile or order the following documents:
Official (original) credit transcripts; these are statements that document the
applicant’s performance in all prior coursework and grades (or marks). The
transcript must be in English or officially translated. The content of the
transcripts must be officially evaluated by one of the following credentials evaluators:

Order: University and Post-Secondary Course-by-Course and Grade Point Average Equivalent
Cost: $130 + $40 = $170
The secondary school or university degree must be accepted as valid for admission purposes and any
coursework taken at an institution outside of Florida must be acceptable as transfer credit by the institution
in Florida to which you are admitted before the tuition exemption is awarded.
A legible copy of the Admission Letter from the Florida institution (SUS or community college)
that is listed in the submitted application
A copy of the passport page that clearly shows the applicant’s name and country of citizenship
A clear photocopy of the applicant’s student’s visa. A legible copy of the immigration papers
received from the admitting university in Florida (either Form I-20 or DS 2019) will also be accepted
for the original award.
A signed letter of recommendation
A standard (4 1/8" x 9 1/2"), self-addressed,
stamped envelope (if the applicant is
from outside of the USA the envelope should not be stamped.)
Typed Statements:
Personal Statement: Please describe your background, personal and
academic interests, academic record and achievements, as well as your aspirations for the future. (approx. 1000 words)
Statement of Purpose: Compose an essay (3-5 paragraphs) describing how your training and
education will contribute to the linkage between your home country and Florida. (approx. 1000 words)
